Bench Show Form Question

As an newly licensed Bench Show Judge I have a question regarding the paperwork portion of the shows.

Why does the official show report form ask for the Handlers City and State when none of the entry sheets, nicely provided by UKC at the links below, not ask for these two items?

that's a very good question, the first link does have a place for city & state. and if its a big show and you have to mail them the receipts, you have no address either. and one thing that bugs me is when ppl fill out the forms and put their addresses...they forget the zip codes!!!

Good question. Handler information other than name is really not necessary. We only ask for owner address for the rare situation we need to contact an owner and they have moved and not kept their registration records up to date.

So, can we leave that blank or just put same as owner?
