How Do I get an Offspring Report. Is there a form that I need to fill out or do I just send a check for the $25 and a letter requesting it with the dogs Regsitration # so they can locate??

Yes, that is exactly what you should do. We do not have a form to request an offspring report, so merely send a written request indicating the name and UKC number of the dog you wish us to do the report on and send it to our office with $25.00.
