If a dog's original registration certificate has been lost, stolen or destroyed, the dog's current Owner of Record can submit an "Application for Duplicate Registration Papers" to request replacement papers. This application is available on our website at: http://www.ukcdogs.com/WebSite.nsf/...onFeesAndForms. In the case of duplicating an Application for Permanent Registration, the Breeder of record can submit the application to duplicate.
We will require that you provide the specific UKC number of the dog for which you need a Duplicate certificate. If your dog's UKC number is not available, please provide as much information as possible to help us confirm the identity of your dog. Once a Duplicate registration certificate is created for that dog, the original certificate becomes VOID and will not be processed. Please note that only the registered owner (breeder) of a dog may apply for duplicate papers.
