 Registry papers

Hello, I bought a dog from a person who filled out the top portion of the registry papers with her name and address. Is there anyway to get new papers that are blank so her name is not on the documentation? I have her papers

You just need to fill out one of our Affidavit forms, which you can download from our website. You'll send the Affidavit in with the registration papers.
