I need to know where to enclose payment information for a change of address? I got the change of address form off the website, but there is NOWHERE to enter payment information to also change the address on my hound's paperwork. HELP!

There is no charge to change the address in the system. If you are wanting your copy of the papers to show your new address, you would have to mail them in for an updated set.

So what am I supposed to do if I do not have the original UKC papers. They got lost in a move....

If you send in this form with the change of address form, they can update the dog's paperwork and get the new set out to you.
