If I messed up on the pup's name while filling out the registration form, can I attach a copy of what I want the pup's name to be?

I would suggest that you draw a thin line through the name on the certificate and then write above it. Do not erasure, use white out, etc. We need to see what you are correcting or an affidavit would be required. This is the only alteration we will allow.
