 Changing address

I just moved and I know you got to fill out a change of address form and send it in also that it's free to not get new papers but my question is if I want the new papers did i send in the ones I have with my old address on them with the change of address form and how do I check if I want a 4 or 3 generation pedigree

Yes you need to send the old papers in.
Yes include the change of address form.
And then simply include the fee for the papers / pedigree you want with your new address. Nothing to "check" and you do not need to sign the old papers. If it were me I would include a note that says not a transfer, update papers only.
