Rimadyl is a NSAID. It was one of the earlier NSAIDS to market so it's not as selective as the newer NSAIDS for Cox-2 receptors. It still works good in many patients though. It has both anti inflammatory and pain effects as do all the NSAIDS on the market now.
Corticosteroids are not that effective in treating arthritis in dogs unless given in the affected joint. They have no direct pain relief action and only work to reduce inflammation. They have too many negative effects on the body to be used long term.
I recommend using multi modal therapy when treating osteoarthritis (OA) in dogs. A combination of an NSAID and narcotic type pain medication seems to work the best. Tramadol is commonly used along with a NSAID like Rimadyl , Previcox, Deramaxx, Metacam, or Onsior to get better control of both the inflammation and pain. Gabapentin works well in many patients as well in combination with the above. Adding 3 or more medications helps to create a synergistic effect for treating OA. Glucosamine/MSM probably isn't going to help much in this stage, but it's not going to hurt.
There are some good veterinarians out there that work specifically with osteoarthritis and pain in dogs. I'd recommend finding someone in your area that is good with that and ask them about what I've written above about multimodal pain control. If they've never heard of it then move on to someone who has.
L-S disease is common in large breed dogs. That is where the lumbar vertebrae meets the sacrum and it is a common place for OA to develop. These dogs can be resistant to medical therapy and respond sometimes to other modes such as cold laser therapy, swim therapy, accupuncture, etc.. An epidural injection of a steroid combined with a narcotic sometimes makes a big difference in these cases as well.
Swim therapy and underwater treadmills work miracles on some of these dogs as does acupuncture where it is available.
