i have 2 dogs given to me. the people done sign their name on the paper but decided not to send it in so paper was given to me. what can i do to put them in my name? it says not to white it out. i hate to white out their name and address it says not to do it. please let me know what i need to do. thanks

For each dog you will need to fill out one of our Affidavit forms. These forms can be downloaded from our website.
