What is the process for paying up a pup under 6 months old for the program is there a form to fill out, cost, etc?

The breeder has to first have the litter nominated at a fee of $25.Once thats been done just call UKC and give them your pup's UKC# and you can pay it up for $25 if under 6mths with a debit/credit card.If over 6mths and up to a year it will be $50.If you have'nt reg. your pup yet just send it in with your puppy papers.
