No appointment neccesary... lead dog in, tech draws blood, walk out. Tech calls you with results, usually next day.
Google is a great thing...all the dosing info you need. Did you try Google? Of course if you can't understand it then I guess that you will have to pay someone else to do it for you.
What do you think the vet is doing? If the level is low, you give more. If the level is high, you give less. It is not exactly rocket science. You are treating symptoms anyway. Start with a minimal amount and increase until your dog is doing well.
