You can submit an "Application for Duplicate Registration" to request replacement papers, which is available on our website at http://www.ukcdogs.com/res/pdf/dupapp.pdf . If the papers were lost on the way to you from our office, please write, "LOST IN MAIL, NEVER RECEIVED" in the area for providing credit card payment. We will process the replacement papers at no charge given that we receive your Application for Duplicate Registration within six months of the date we originally issued your paperwork. Additionally, please know that you are welcome to fax your "lost in mail" request to us at 269-343-7037 at no additional cost.

We also want to mention that if your puppy paper was lost in the mail on the way to our office, and was still in the breeders name, you will need to ask the breeder to request the Duplicate paperwork, as we will only duplicate an application to the owner on record.
