problem... lost registration paper:(

I recieved the ukc temporary registration certificate from my breeder friend. Problem is.. I moved.. then became very ill with cancer. I couldnt find the papers and had no way of contacting her. Since then I have found them. Is there any way for me to get my dog re registered?
Thank you so much for your time and help!

Thank you for your question. Please download our Application to Re-Establish a Cancelled Record at http://www.ukcdogs.com/rg/pdf/affidavit.pdf You will need to download the latest version of Adobe Acrobat Reader if you haven't already. If you have any trouble, email me your name and address and the name of the form (Application to Re-Establish a Cancelled Record) at sjonas@ukcdogs.com and I will send you one snail mail.
