Simply fill out the Bill Of Sale on the reverse side of the Application for Permanent Registration as such:
- Date of Sale
- New Owner: your name, and co-owner name, if applicable
- New Owner sign name: your signature
- New Co-Owner (if any): if applicable, co-owner’s signature
- If in partnership, check ‘Yes’ or ‘No’ to the question. If not in partnership, disregard.
- New Owner’s Address: your street address or postal box
- City: your city
- State: your state or region, also include country information
- Zip Code: your postal code
- Phone: your phone number
- E-mail: your e-mail address, if any
- Name dog here: the name must be at least two words, not over 30 characters (including spaces and punctuation). If your dog is eligible for ‘Purple Ribbon’ or ‘PR’ designation, we will include that, there’s no need to include that in your dog’s name.
- Seller’s Signature: we must have the signature of the person (or people if in an “and” partnership) whose name appears on the front of the certificate.
Select one of the following at the top:
- $16 – Registration to new owner & Three (3) Generation Pedigree
- $35 – Registration to new owner & Six (6) Generation Pedigree
- $50 – Registration to new owner & Seven (7) Generation Pedigree (Parents of dog must be ‘PR’)
And, if you want faster service:
- $8 – Rush Service – papers will be processed within 5 business days after we receive them. Make sure to write RUSH on the front of the envelope.
Send a check (personal checks must have the words “Payable In U.S. Funds”) or money order (payable/negotiable in U.S. funds), along with the completed Application for Permanent Registration to:
