If a dog's original registration certificate has been lost, stolen or destroyed, the dog's current Owner of Record can submit an "Application for Duplicate Registration Papers" to request replacement papers. This application is available on our website at: http://www.ukcdogs.com/WebSite.nsf/...onFeesAndForms.

We will require that you provide the specific UKC number of the dog for which you need a Duplicate certificate. Once a Duplicate registration certificate is created for that dog, the original certificate becomes VOID and will not be processed. Please note that only the registered owner of a dog may apply for duplicate papers.

Thank you again for contacting us. Please feel free to contact our office at 269-343-9020 if you have any further questions.

What if the current registered owner is deceased?

Then the executor of their estate would have to apply for duplicate papers. They would have to send in the duplicate application along with a copy of the registered owner's death certificate and something officially showing that they are in fact the executor of the estate.
