Form for Address Change?

I need a form to fill out payment information to change address AND change address on my dog's registration paperwork. I got the change of address form online, but there is nowhere to enter payment info on that form. I want to change the address on my dog's pedigree.

There is no fee to do a change of address. Complete the form, sign, and date and submit.

I got that. But, I want the address change to reflect on the UKC papers, which got lost in my last move.
