Should a guy be concerned about female getting mastitis with only 2 pups on her?

Yes. That milk can become toxic and she should be put on antibiotics. There are several that are safe for nursing pups. She can nurse from the unaffected teats but you may need to switch them to hand feeding from a bottle.

For a dog with mastitis, the recommendation is to try and express the milk from the infected gland, do it until the teat returns to normal. You want to get the infection out. You actually can allow the pups to nurse but most won't. Antibiotics are needed and you can also use cold or frozen cabbage leaves on the affected teat. Pack on, hold on until the leaves are warm then remove and replace with fresh cold ones. Shouldn't need to bottle feed the pups unless it's indicated by the type of antibiotic used. Some are not good for young pups.

Absolutely not. I have had females have singlets and never have a problem. Clean bedding, water, and a healthy pup is all she needs. I have seen more females with huge litters develop mastitis than small litters. If you are worried consult your vet rather than a bunch of people you do not know.
