request for statement of account

Hello,

i want to request who owners or dog name that i paid
under my name ALFRED G. CORTES
I WAS BIILED from my bank LAST

JUNE 06 21 USD 187
JUNE 08 21 USD 49
JUNE 08 21 USD 30
JUNE 08 21 USD 49
JUNE 08 21 USD 49

so i can see the breakdown of charges including name of owners.
individual owners did not receive papers until now.
