Lost puppy papers?

We just got a dog back that was sold as a puppy, the owner didn't send in the papers and lost them. The dog is now almost 2 years old, what do we need to do to register him? Thanks

The breeder would have to fill out one of our duplicate applications which can be downloaded from our website. They can do the permanent registration to themselves off that form.
