Bench Show Judge

Hello my club has a member looking to become a bench show judge, I know there used to be a test and paperwork for the club officials to fill out online. I looked but didn't see it. Has the process changed or am I just missing where to find the paperwork?

I believe you have to call the Hunting Programs Department and request a test and application be sent. Then the application is returned with the completed test. Should they pass, UKC will send apprenticeship forms.
