Litter Registration question

I have a one pup litter (English Coonhound) that I will be registering down the road, and plan on paying the performance up at the time of registration and getting a seven generation pedigree as well at that time... anyway the Litter Registration Application that the sire owner has signed doesn't have the performance stuff on it! Can I just use that application and drop a note with it adding the performance payment to the totals, or can I print out a new litter application with all the performance options and fees on it and putting this one with it for the sire information as it is signed, or will I have to have him sign a get me another litter application that has all the performance options and fees on it? Thank you and hopefully the question makes sense...lol...

Just include a note explaining everything you want to do and that will be fine.
