Permanent Registration Changes

I have my dogs permanent registration paperwork. The problem is I filled it out when she was 8 weeks old. Her color has since change. Along with my address & credit card # I wrote on the paper. Can I use white out & change it all if I print & fill out a affidavit form stating why is was changed? Thanks in advance!

You don't need an Affidavit. You can white-out and change the color information and the credit card information. For the address, just include a written, signed statement on a separate piece of paper stating that after you filled the papers out you have moved. Give us you complete new address and ask us to send the papers to your new address. Sign & date the statement and include it with the papers and you'll be fine.
