Name change question

My pup’s breeder decided to fill out the name on his registration before I purchased him.
I did go ahead & register him after finding out I could change the name after he was owned by me.
Unfortunately the name they gave him isn’t the name I have given him & there are some spelling issues as well.
I know that I can put the correct name on the back of the registration & send it back to change it.
What I can’t find is a box to check listing the change & fee to have it corrected.
Please advise.

Just fill out the new name for the dog and mark which pedigree package you would like with the new papers. That fee is the fee you will need to pay.
