Registration paperwork question

I have just picked up a UKC registered 2 y/o redbone coonhound that my uncle has given me because of his busy work schedule. He bought the dog and has not put her in his name yet but he filled out most of the back the pedigree with intensions on sending them into the ukc but he forgot to. Now heres my problem I have the dog now and im looking to put her into my name what should i do with the registration that is almost completely filled out???? Can i white out all the filled out information, fill in my information and mail the paperwork back in or do i need to have my uncle complete what he wrote send the paperwork in to get a pedigree with a clean registration form on the back?

You will need to fill out an additional form we have called an Affidavit. I can mail this form to you or you can download it off our website.

Now when I mail in the Affidavit form will I receive an updated easy entry card with my information and a new updated three generation pedigree ?

You'll send in the Affidavit with the registration papers, and yes, you'll receive new papers and a new EE card.
