i have forgot the policy for how these are handled, for example; how long does a scratch stay on record, if a dog was scratched april 1st of last year, and then again april 18th of this year how many are on record? is 3 in one year then the dog is barred or does it not matter?

They are barred if they are scratched 3 times within any 2 year period. So a scratch stays on for 24 months and then it comes off. In your example, the dog would be barred if scratched a third time before April 1st of 2014.
