Lost Papers Please Help

I HAVE LOST A SET OF PAPERS IN MY MOVE AFTER I BOUGHT MY HOUSE I HAVE SEARCHED EVERY WHERE FOR THEM AND NO LUCK CAN I IN ANYWAYS GET ANOTHER SET ON HER ANY INFO WOULD BE GREAT HER NAME IS LATE NITE NIKKI AND MY NAME IS DAVID POFFENBERGER

Assuming the dog is registered in your name, you just need to fill out one of our duplicate applications. This form can be downloaded from our website, or you can send me a PM with your name & complete mailing address and the fact that you need a duplicate application and I can mail you one.
