I had the same problem with one of my hounds. He had a slight yeast infection and the vet gave me some sweet pea ear rinse and some kind of drops I had to put in everyday. I would go to another vet.
I have one hound with a very loud mouth that barks so loud he shakes his head afterwards. This has resulted in bald spots on the tops of his ears, and occasional splits in his ear tips, especially in winter.
He is now 8 years old and we've given up trying to stop this. We tried every conceivable kind of ear remedy, we clean his ears regularly, and there is no mite or yeast problem.
The bald ears look ugly and the split tips bleed some, but it doesn't cause any serious harm, and I love his LOUD mouth....
