Registering a 5 year old

I have a 5 year old American Rat Terrier. When we purchased him 5 years ago the breeder gave us the papers to send in to register him with the UKC (his parents are both registered UKC). I have looked at the Registration link and do not understand what I need to fill out or if I can download (I do have Adobe), fees and anything else I may need to do. Please advise me on this.........The reason Brodie was never registered is a mystery. We just forgot and ran across the unsent papers recently!

If you still have his temporary registration certificate (puppy paper), you should complete the Application to Re-establish a Cancelled Record and submit it with the registration certificate. You can download this form from the website, or if you e-mail me with your address I will be happy to mail the form to you.
