The registered owner of the dog would need to fill out one of our Application for Duplicate Registration forms. This form can be printed out from our website.

Just fill out as much of the duplicate application as you can and we'll try to fill in the rest of the information once we receive the application.

You can send me a PM with your name, complete mailing address, and the fact that you need a duplicate application and I can mail it to you, or you can try to print if out from a different computer.
