Messed up paperwork

When filling out my dogs name on the papers I messed up the name. What do I need to do now to get another set of papers to fill out so I can register this pup.

Just include a written, signed statement telling us exactly what the mistake was and what you want the dog's name to be. Do NOT cross out or erase or white out the name that you put on the papers.
