pup registration

i bought a pup and got her paper to send in but messed up on the name of the pup can i get another paper i have ukc numbers for mother and father and puppy

You need to fill out an additional form called an Affidavit. You can download the Affidavit from our website, fill it out and send it in along with the registration papers.
