Russel~ frozen semen litter

We are going to be doing a frozen semen litter from AKC/UKC stud and dam. I have the AKC frozen semen litter registration and a litter registration for UKC. My question is:
Is there a seperate litter reg. form for frozen A.I litters or do you just use the reg. litter reg papers and fill in those 2-3 lines on that form. Reason I ask is that AKC makes things so complicated at times and I want to make sure that all my paperwork is complete and correct so that the pups will be reg. before going to new homes.

Just use the regular litter registration form and be sure to fill in the A.I. portion of the applicaiton.

Also, since it's frozen semen we do require that we have the Sire's DNA on record.

You should have it, sire owner UKC registers all their dogs and have used this sire for their own dogs since his passing (deceased). Do you need the females DNA as well? I think I have her AKC profile from MMI but I don't believe I transferred it to UKC as well.

No, we do not require the Dam's DNA.
