I insert 1/2 - 3/4 cc in each ear once a month to prevent them and it works great. Not sure how much or how often is needed to treat them.
What worked for me
I don't know about in the ear, but 3/10 cc injected under the skin or mixed with pop and shot down their mouth worked for me for pups.
I also had a male dog that weighed about 80lbs that I gave 1 cc by mouth.
I repeated this once a week for 3 weeks. Cleared everything up for me.

ear mites
Give oraly for heart worms. Not more than one cc. Any thing over that would be a mange dose. I usally give 1/2 to 3/4 depending on the size of the dog. No need to dilute it down with anything. Just put it in a syringe(no needle) and shoot it down the throat.
For ear mites the vet gave me a little plastic dropper it is about a 1/4 cc in size give 3 drops in both ears then repeat in 2 weeks. Then once a month put one drop in each ear for prevention.
Once a month I'll give'm all a round. It is a heck of a lot cheaper than buying the pills which are ivermectin anyhow. Also do not buy the larger bottle's like you would for feeder pigs it is not the same strength. Good luck!
