Performance eligibility forms? / pup registration

Any help appreciated

I didn't receive the performance application forms with the pup registration forms. I have been unsuccessful contacting UKC or getting an email response. I have not been able to find the forms online. Any suggestions? Are the forms online now? Did they just neglect to include them?
