Lost Puppy Papers

I have lost the puppy papers on a 4 month old pup before I had a chance to send them in. I have the puppies UKC number and the sire and dams number. Which form do I use or how do I go about getting the papers straightened out.

In case it helps the pups number is W331,893

The breeder needs to fill out the Application for Duplicate Registration form and send it in to us. We will issue them another set of papers and they can sign them over to you, just as they did the original set.
