Papers Question

Hello, I sent some papers in a while back, I made an error when I filled them out. UKC told me to fill out an affidavid and send with them. A few weeks later i received a letter wanting me to fill in my name and address again, so I sent it in. Just making sure everything is in order. PR POPLAR SPRINGS POISON IVY Thank you, Abe Lucey
