Duplicate Papers

I requested a duplicate form and recieved it. I have the hounds UKC number, but can't remember much else it requests on the form. Such as DOB, And some of the other questions. Is the UKC number enough on that form? Or do I have to try to fill it out completely? Thanks

The most important information needed to get duplicate papers is your name and address and the name and UKC number of your dog. But the other information we request is also important so we can be sure we have the correct dog--if all information matches, we can be sure you will receive the correct papers.
