Registration question

Trying again to get registration papers on our cocker spaniel. Bought him as a puppy a few years back. Received a litter shot record having his UKC ID# and Sire and Dam's name and UKC#. Through the maize of applications, we don't know what we are doing. What paperwork do we need to fill out to get papers on him? Thanks in advance.
