points problems

I just noticed while reviewing my dogs records today that of 4/25/15 he was awarded 20 pts, however the UKC Reg. number on the slip was incorrect. The dogs name is CH PR Fellow Flyhigh Barney his actual registration number is W179,001 but on the slip it was written in 193,001 how does this get corrected?
